*Requires authorization/referral from PCP

As a service to patients, our office will bill the patient’s insurance (both primary and secondary) for services rendered. Co-payments and payments for treatment not covered by insurance are due at time of service. Additional charges, co-payments, co-insurance and deductibles are required at the time of each visit.
